Physical Forms and Athletic Releases
Each athlete must have a completed PIAA Physical Form in the hands of the Head Coach and a copy on file with the Athletic Director's Office. Along with these forms there is additional release forms that must be signed by both parents and athlete. With Swimming, Diving, and Water Polo we always have the exact current forms necessary in the pool office. Please use these forms only. Also, prior to the start of each season, the school offers free physicals done by a school physcian. You can take advantage of this opportunity or you can go to the Doctor of your choice. Again, when you go to any physical, please have the forms completed as much as possible so that the Doctor can complete his/her portion correctly.

What to do when or if you have been injured:
Wyoming Valley West offers one of the most competent injury prevention and injury care programs in the state or possibly country. Coach Fred is our athletic trainer and is responsible for all injury care. When an injury occurs you have a choice of going to the Doctor of your choice or you can ask Coach Fred for any suggestions. His job is the immediate care of an injury, the rehabilitation of an injury (all consultation is done with the primary physician), and along with the Doctor the decision if it is safe for the athlete to return to play.
Please, if there is an injury, make sure that you report to Coach Fred immediately so that if necessary any and all school insurance will support your personal primary insurance. What we need everyone to understan is that when injured your insurance covers all injuries with Doctors and Hospitals. The school insurance when reported correctly to Coach Fred will pay for any additional costs billed to you.
Insurance injury procedure: Report and care for injury, report injury to the coach and Coach Fred. Submit any additional billing beyond your insurance to coach Fred.
Impact Testing: This program has been and continues to be in placed supported by the Wyoming Valley West School District. Each athlete needs to have a baseline evaluation. We have all ready completed our testing. If any other aquatic athlete has not completed this baseline test, please see coach Tribendis immediately.
